Forms for Families

Below is a list of commonly used form for child care clients. Please contact your case worker at Connect CCR&R to ensure that you are using the proper forms.

Form Use
30-Day Job Search Form Used to request a job search after loss of employment
Change of Information Notification Form Used to change any information regarding your child care case such as change in address, change in child care provider, etc.
Child Emergency Information Form Given to child care provider for every child in care
Child Care Review Form/Status Check Used to renew client's child care certificate every 6 months
Child Medical Form Given to child care provider for every child under the age of 5
Household Occupancy Form Remove a biological parent or spouse from the home. Forms must be notarized and two must be submitted from persons other than relatives.
New Employment Verification Form Used to verify employment within the last 45 days
Self Employment Ledger Used to track income and expenses for self employed clients
Release of Information Form Gives Connect permission to share information about the clients case with others (Mother, Father or Grantparent etc.)
